What are some of the most popular ways fillers are used today?
Dr. Collins: The goal with fillers today is to achieve a natural refreshed look to rejuvenate appearance. Subtlety is in, fake is out.
Fillers are being used to correct midface volume loss. We anchor the skin in the middle of the face to lift up and out to improve sagging jowls and reduce wrinkles around the mouth. Today, we have a deeper understanding of the aging face and by using fillers to address this can create a more natural appearance.
The most popular fillers are the hyaluronic acid fillers. The newest versions of these fillers now come in a variety of molecule sizes. These new variations allow us to treat more areas and address problems more effectively.
Larger molecules help with volume enhancement in areas like the cheeks; smaller molecules help with fine lines and wrinkles. There are also new uses for fillers, including:
- Chin augmentation
- Buttock augmentation
- Correction of temporal hollowing
- Ear lobe augmentation
- Hand rejuvenation
- Treatment of acne scars

Dr. Kaufman: Newer products have allowed us to do even more with fillers. Newer fillers can help build volume deep in the cheeks while some softer ones can be used more superficially or in the lips without the lumpiness experienced with previous fillers.
Fillers are frequently used to fill winkles of the face. But they can also be used to introduce volume to areas that have lost fullness due to weight fluctuations, aging and gravity. More recently, they have also been used for "liquid rhinoplasty," where the appearance of the nose can be improved without surgery.
Dr. Mir: The most popular areas remain the smile lines (nasolabial folds) and the marionette lines (lines extending down from the corners of the mouth). The hollowing/shadows under the eyes have become very popular as well.
The new filler Voluma® is becoming increasingly popular. It is designed to restore volume to the cheeks. This creates a more youthful appearance by lifting and supporting the cheek and also helps the nasolabial folds and jawline for lower face enhancement, as well as the "tear trough" area at the same time, and lasts up to two years.
Who is an ideal candidate?
Dr. Collins: We're seeing a lot of women in their 60s who aren't interested in invasive procedures like face-lifts or they had a face-lift in their 50s and don't want another one. These patients are instead choosing fillers to correct some of the lines.
Dr. Kaufman: Anyone who has wrinkles or has loss of volume in the lips or cheeks is a great candidate. People who shouldn't get fillers, including anyone on blood thinners or individuals with multiple allergies.
Dr. Mir: Ideal candidates have moderate wrinkles and folds, good skin quality and elasticity. It's best to come in for a consult to discuss all of the options.
