Caring for Kelsey-Seybold patients at hospitals throughout the Houston area.

Kelsey-Seybold hospitalists (including pediatric hospitalists) provide specialized care for our hospitalized patients at several hospitals throughout the Greater Houston area. These include St. Luke’s Health – Baylor St. Luke’s Medical Center, Texas Children’s Hospital, and The Woman’s Hospital of Texas. We also have a vast network of affiliate physicians who take care of Kelsey-Seybold patients at many other Houston-area hospitals.

Hospitalists are physicians who take over the care of the patient from the primary care physician (PCP) while the patient is in the hospital. The hospitalist’s role is to ensure that each patient receives the level of care that’s required for the level of their condition. They observe and evaluate the patient’s status, progress, and test results, which allows them to provide evidence-based, personal, patient-centered care. Through hands-on interaction and an understanding of the patient’s medical history, a hospitalist gets to know each patient’s unique needs during their hospital stay and coordinates their care with their PCP and specialists.

Once the patient is discharged, the hospitalist ensures the patient is returned to the care of their primary care physician and that all information related to the hospital stay and medication changes are included in the patient’s electronic medical record.

Benefits of Hospitalist Care

When a Kelsey-Seybold patient is admitted for hospitalization, the Kelsey-Seybold hospitalist acts as the “team captain” for the patient’s care by:

1. Evaluating and treating the patient in the ER, when necessary, in collaboration with the ER physician.
2. Initiating the patient’s Plan of Care.
3. Ordering specialist consults as needed from Kelsey-Seybold or the clinic’s affiliate network.
4. Expediting testing, reviewing results, and coordinating in-patient care with specialists on the care team.
5. Serving as the main point of contact for the patient’s primary care physician and specialists during the hospital stay.
6. Continuously identifying opportunities for transfer to the next level of care appropriate for the patient’s condition.
7. Arranging primary care physician and specialist follow-ups and determining appropriate level of post-acute care, if needed, upon discharge.
8. Alerting the Kelsey-Seybold nurse case manager or social worker if the patient requires further assistance at home after discharge.
